A unique opportunity is available for a Nurse Practitioner specializing in Home Health to perform risk assessments for Medicare Advantage members throughout Lake County. This per diem position is ideal for professionals seeking flexibility and autonomy while making a meaningful impact. You will complete comprehensive assessmentseach lasting about 45 minutesat patients homes, possibly involving annual visits or post-discharge evaluations. Assessments may take you to neighborhoods such as Gary, Hammond, and Merrillville, and scheduling is designed to support your work-life balance.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hourscreate your ideal schedule
- Choose a minimum commitment by week or bi-week
Key Responsibilities:
- Perform in-depth home health risk assessments for Medicare Advantage plan members
- Assess and document prior/current medical history, ongoing conditions, and medication usage
- Provide patient education regarding health conditions and medication regimens
- Identify potential gaps in care and recommend follow-up solutions
- No requirement to develop treatment plans or prescribe medications
Desired Qualifications:
- Current Nurse Practitioner License in Indiana
- Certified in Basic Life Support (CPR/BLS)
- Board Certification from AANP or ANCC
- Minimum 6 months of current Nurse Practitioner experience preferred
- MSN degree from an accredited nursing school
