This position offers the opportunity to conduct home health risk assessments for Medicare Advantage members throughout Calhoun County, Texas, including Port Lavaca, Seadrift, and surrounding communities.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule to fit your availability
• Commitment to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments, averaging 45 minutes per patient
• Review patients prior and current medical history, conditions, and medication usage
• Educate patients on managing chronic conditions and understanding medications
• Identify gaps in care and recommend appropriate actions to improve health outcomes
• No requirement for formulating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Texas
• Board Certified through AANP or ANCC
• Current Basic Life Support (BLS/CPR) certification
• Master of Science in Nursing (MSN) from an accredited program
• A minimum of 6 months recent experience as a nurse practitioner is preferred
