Responsibilities include conducting comprehensive home health risk assessments for Medicare Advantage plan members across Erie County, OHincluding areas such as Sandusky, Huron, and Vermilion. Each visit, typically lasting around 45 minutes, may involve annual health reviews or post-discharge assessments. Youll focus on evaluating medical histories, current conditions, and medication usage. Educating patients about their health, identifying care gaps, and recommending next steps without the need to create treatment plans or prescribe medications are key aspects of the role.
Qualifications:
- Active Nurse Practitioner License in Ohio
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC required
- Minimum of 6 months recent NP experience (preferred)
- MSN from an accredited nursing program
