Make a significant impact while enjoying unmatched flexibility as a Nurse Practitioner delivering Home Health risk assessments on a per diem basis. This role involves visiting Medicare Advantage plan members in Sandpoint, Ponderay, Priest River, and other communities throughout Bonner County, conducting comprehensive evaluations in the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule and determine your workload
- Options for both minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform annual health risk evaluations for Medicare Advantage members in their homes
- Review and assess patients prior and current medical history, ongoing conditions, and medication usage
- Educate patients about existing medical conditions and their medications
- Identify care gaps and recommend solutions to promote overall health
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current CPR certification (BLS)
- At least 6 months of recent nurse practitioner experience preferred
