Job Description
Serve as a valuable resource to patients in Kay County by conducting comprehensive home health risk assessments for Medicare Advantage plan members. This per diem opportunity is ideal for nurse practitioners seeking flexibility, autonomy, and meaningful patient interactions across locations such as Ponca City, Blackwell, Newkirk, and Tonkawa.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to accommodate your preferred days and hours
- Commitment can be set on a weekly or bi-weekly basis
Primary Responsibilities:
- Complete home-based annual risk assessments for Medicare Advantage plan members
- Assess medical history, review ongoing conditions, and reconcile medication lists
- Educate patients on their health conditions and proper medication usage
- Identify care gaps and recommend next steps for patient follow-up
- Each assessment typically lasts approximately 45 minutes
- No requirement to create ongoing treatment plans or prescribe medications
Required Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN from an accredited nursing program
- At least 6 months of current nurse practitioner experience preferred
