Looking for a meaningful opportunity to utilize your clinical expertise and make a difference in patients lives? We are seeking an experienced Nurse Practitioner to conduct home health risk assessments for Medicare Advantage plan members across Broward County. This per diem position offers unmatched scheduling flexibility and the chance to impact overall patient wellness directly within their own homes.
Schedule and Hours:
- Flexible shifts available seven days a week
- Ability to set your own schedule
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive annual home health risk assessments
- Review and evaluate patients prior and current medical histories, including medical conditions and medication use
- Educate patients about their health conditions, prescribed medications, and preventive healthcare
- Identify gaps in care and recommend solutions to enhance patient outcomes
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Active and unencumbered Nurse Practitioner license in Florida
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
Your work will involve seeing patients in locations throughout Broward County, with each assessment typically taking around 45 minutes. Areas may include Fort Lauderdale, Pembroke Pines, Hollywood, and surrounding communities.
This opportunity features compensation of $80-$90 per evaluation, along with a mileage stipend and additional compensation for administrative time.
