A rewarding opportunity is available for a Nurse Practitioner specializing in home health, focused on conducting risk assessments for Medicare Advantage plan members throughout Meigs County, Ohio. This per diem position allows for flexible shifts and locations, with visits scheduled in Pomeroy, Syracuse, Racine, Middleport, and nearby areas. Ideal candidates are experienced in home health assessment and eager to provide quality care in a community setting.
Schedule:
- Flexible shifts available between Monday and Sunday
- Set your own schedule to fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior/current medical history, conditions, and medication usage
- Deliver education to members regarding their conditions and medications
- Identify care gaps and offer recommendations for resolution
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Ohio
- Current CPR/BLS certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited institution
- At least 6 months of current nurse practitioner experience strongly preferred
Compensation for this role is $80-$90 for each evaluation, along with a mileage stipend and additional pay for administrative tasks.
