If you are a Nurse Practitioner passionate about patient assessment and education, this per diem opportunity could be an excellent fit. In this role, you'll perform home health risk assessments for Medicare Advantage plan members in Le Flore County. Assessment visits typically last about 45 minutes and may range from comprehensive annual assessments to post-discharge follow-ups.

Schedule:

• Flexible shifts available any day, Monday through Sunday

• Commitment to minimum weekly or bi-weekly hours

Key Responsibilities:

• Conduct thorough home health risk assessments for Medicare Advantage members
• Review and document past and current medical history, chronic conditions, and medication use
• Educate patients on their medical conditions and provide medication education
• Detect gaps in care and recommend solutions to enhance patient well-being
• No requirement to manage ongoing treatment or prescribe medications

Qualifications:

• Valid Nurse Practitioner license in OK
• CPR certification (BLS)
• Board certification via AANP or ANCC
• At least six months of recent NP experience preferred
• Master of Science in Nursing from an accredited program

This position offers competitive rates, with compensation of $80-$90 per completed evaluation, plus a mileage stipend and paid administrative time.
