Deliver a rewarding impact in the community by conducting vital home health risk assessments for Medicare Advantage plan members across Washington County, NC. As a Nurse Practitioner specializing in home health, you will travel to members homes in communities including Plymouth, Roper, and Creswell, taking part in meaningful encounters that help to ensure continued well-being and quality of care.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Create your own schedule according to your availability
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Perform comprehensive home health risk assessments, including annual and post-discharge visits
• Evaluate medical history, existing conditions, and medication usage for each patient
• Educate patients and their families on medical conditions and appropriate medication use
• Identify care gaps and recommend practical solutions, enhancing patient outcomes
• No need to develop treatment plans or prescribe medications, focusing on assessment and education

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification from AANP or ANCC
• MSN degree from an accredited nursing program
• Current CPR certification (BLS)
• At least 6 months of nurse practitioner experience is preferred

In this per diem role, you will receive $80-$90 per completed evaluation in addition to a mileage stipend and compensation for administrative time.
