Bring your expertise as a Nurse Practitioner to a dynamic per diem role focused on home health risk assessments within Washington County, NC. Schedule:
- Flexible shifts available Monday through Sunday

- Set your own minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Assess medical history, chronic conditions, and medication usage
- Educate patients regarding their medical conditions and medication regimens
- Identify and document care gaps, offering practical recommendations for improvement
- Focus solely on assessmentno medication prescribing or treatment planning involved
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certified through AANP or ANCC
- Current BLS/CPR certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least six months of current NP experience preferred
