Job Description

Deliver valuable care to the community as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage members in Washington County, NC, including Plymouth, Roper, and Creswell. This per diem opportunity allows you to support preventive health while enjoying the flexibility to structure your work around your life.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members in clients residences
  • Evaluate past and current medical history, chronic conditions, and medication regimens
  • Offer patient education on medical conditions, disease management, and home safety
  • Identify gaps in care, providing actionable recommendations for follow-up
  • Assessments may include annual check-ups or post-discharge visitsno treatment planning or prescribing required

Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • Board Certified through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN from an accredited nursing program
  • Minimum of 6 months recent Nurse Practitioner experience preferred

This role offers $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
