Job Description

This per diem role gives you the autonomy to set your own schedule with flexible shifts available every day of the week. Assignments take place across various locations within Chowan County, ensuring diversity in your daily responsibilities.

Schedule:

  • Flexible shifts Monday through Sunday
  • Self-scheduling to fit your availability
  • Select a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan participants
  • Conduct thorough evaluations of medical history, current conditions, and medication use
  • Deliver education to patients regarding their conditions and prescribed medications
  • Recognize and document any gaps in patient care, providing actionable recommendations
  • Note: There is no requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Valid and current CPR/BLS certification
  • Board certification through AANP or ANCC
  • A minimum of 6 months of recent Nurse Practitioner experience preferred
  • MSN degree obtained from an accredited nursing program
