Embark on a rewarding per diem role providing home health risk assessments for Medicare Advantage plan members across Montgomery County, TN. As a Nurse Practitioner, you will offer compassionate, professional care through in-home visits in neighborhoods such as Woodlawn, Cunningham, and Sango, tailoring your hours for a work-life balance that fits your needs.
Schedule:
- Flexible shifts available seven days a week
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform thorough annual home health risk assessments during 45-minute in-home appointments
- Review and document patients prior/current medical histories, conditions, and medications
- Offer personalized education on disease management and medication adherence
- Identify and communicate care gaps, suggesting actionable solutions
- Strictly assessment-focused: no prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner preferred
- Masters in Nursing (MSN) from an accredited program
