Experienced Nurse Practitioners with a passion for preventive care can utilize their expertise by delivering in-home health risk assessments throughout Kalamazoo County. This per diem role focuses on supporting Medicare Advantage members through comprehensive, focused health evaluationseach averaging 45 minutesin their residences. Home visits may include annual health assessments or post-discharge check-ins, with flexibility to serve residents in Kalamazoo, Portage, Oshtemo, and the broader county.

Schedule:

• Flexible shifts available between Monday and Sunday
• Create your own work schedule
• Minimum hourly commitment required weekly or bi-weekly

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage members
• Evaluate prior and current medical history, medical conditions, and medication regimens
• Provide education to patients on their health conditions and medications
• Identify gaps in care, offering recommendations for improvement
• Avoid treatment planning or prescribingfocus solely on assessment and education

Qualifications:

• Active Nurse Practitioner (NP) license in Michigan
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC required
• Completion of MSN degree from an accredited nursing school
• At least 6 months of recent NP experience preferred

This assignment offers compensation for each evaluation completed, ranging from $80$90, plus a mileage stipend and payment for administrative time.
