Job Description

Deliver impactful patient care in the community by performing home health risk assessments as a Nurse Practitioner in a dynamic per diem role. In this position, you will visit Medicare Advantage plan members throughout Kalamazoo County to complete detailed evaluations, educate patients, and enhance health outcomes. Schedule:

  • Flexible shifts available seven days a week
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct home health risk assessments, including annual and post-discharge visits
  • Evaluate patients medical history, health conditions, and medication usage during in-home appointments
  • Educate members on their health conditions, care plans, and medications
  • Identify gaps in care and provide recommendations for closing them
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Michigan
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited school of nursing
  • Minimum of 6 months of recent NP experience preferred
