This per diem position offers both flexibility and autonomy, empowering you to craft your own schedule while providing essential risk assessments to patients in the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingchoose the days and hours that best fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Review and document medical histories, current medical conditions, and medication lists
- Educate patients on managing health conditions and adhering to medication regimens
- Identify care gaps and share recommendations to improve patient outcomes
- Focus solely on evaluationsno prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current BLS (CPR) certification
- Board Certification via AANP or ANCC
- Masters Degree in Nursing (MSN) from an accredited institution
- A minimum of 6 months recent NP experience is preferred
