Deliver essential care to Medicare Advantage members as a Nurse Practitioner specializing in home health risk assessments. In this rewarding per diem opportunity, youll visit patients in their residences in Plainfield and surrounding areas within Hendricks County, ensuring access to quality health evaluations and education.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Meet a minimum hourly commitment each week or every two weeks
Core Responsibilities:
- Perform comprehensive annual health assessments for Medicare Advantage plan members in home settings
- Assess medical, medication, and prior/future health information for each individual
- Educate members about chronic conditions, current treatments, and medication adherence
- Spot gaps in care and propose actionable recommendations
- Focus evaluations on risk assessmentno treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board Certification (AANP or ANCC)
- Current BLS/CPR certification
- MSN from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
