Make a meaningful impact by delivering personalized healthcare directly to patients homes. This per diem opportunity is focused on providing comprehensive risk assessments for Medicare Advantage plan members across Pittsburgh and surrounding areas in Allegheny County. Youll conduct in-home evaluations, supporting the continuum of care and helping to improve health outcomes for a diverse group of individuals.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: choose when you work
• Minimal weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete thorough home health risk assessments, typically lasting about 45 minutes per member
• Review and document patients prior and current medical history, conditions, and medication lists
• Educate members on their health conditions and medications in clear, compassionate terms
• Identify gaps in care and provide actionable recommendations to enhance health management
• Visits may be annual health assessments or post-discharge check-ins
• No prescribing or treatment planning requiredfocus is assessment and education

Qualifications:

• Active Nurse Practitioner license in Pennsylvania
• Certified in Basic Life Support (CPR/BLS)
• Board certification through AANP or ANCC
• At least six months of recent nurse practitioner experience (preferred)
• MSN degree from an accredited program

This role offers $80$90 per evaluation, plus mileage stipend and compensation for administrative time.
