Job Description

Step into a rewarding per diem opportunity serving Medicare Advantage plan members by conducting home health risk assessments throughout Transylvania County, including Brevard and Rosman. In this independent Nurse Practitioner role, you will conduct comprehensive evaluations in patients homes, helping to enhance care outcomes while enjoying exceptional flexibility.

Schedule

Key Responsibilities

  • Perform annual home health risk assessments for Medicare Advantage health plan members
  • Review and document patient medical history, medical conditions, and medication usage
  • Deliver patient education regarding diagnosed conditions and prescribed medications
  • Identify gaps in care and make appropriate recommendations for follow-up
  • Focus on assessments onlyno prescribing or treatment plan development

Qualifications

  • Active Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC required
  • Minimum of 6 months recent Nurse Practitioner experience strongly preferred
  • MSN from an accredited nursing program
