Elevate your clinical expertise as a Nurse Practitioner by providing home health risk assessments for Medicare Advantage plan members in Middlesex County, NJ. As a valued per diem team member, youll deliver care to patients where they reside, covering a range of locations including Edison, Woodbridge, and Old Bridge. Assessments generally last about 45 minutes and may involve annual health evaluations or post-discharge visits, supporting better patient outcomes in the community.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review previous and current medical histories, health conditions, and medication lists
• Educate patients about their medical conditions and medications
• Identify gaps in care and recommend appropriate next steps
• Focus solely on assessmentno treatment plan creation or prescription writing required

Qualifications:

• Valid Nurse Practitioner license for the state of New Jersey
• Current CPR certification (BLS)
• Board certification from AANP or ANCC
• At least 6 months of recent NP experience is preferred
• MSN from an accredited nursing school
