This per diem opportunity is ideal for skilled Nurse Practitioners who excel in home health settings and are interested in performing meaningful, focused clinical assessments within Middlesex County. You'll be empowered to manage your own schedule and provide exceptional care to Medicare Advantage plan members residing throughout communities like Edison, Woodbridge, and Old Bridge.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Commitment can be set weekly or bi-weekly

Key Responsibilities:

• Conduct thorough home health risk assessments for Medicare Advantage members
• Evaluate medical histories, current conditions, and medication usage
• Educate patients about their diagnoses, treatment regimens, and medication management
• Identify gaps in care and provide actionable recommendations to improve health outcomes
• Focus exclusively on assessmentsno treatment planning or prescribing required

Desired Qualifications:

• Active Nurse Practitioner license within New Jersey
• Current Basic Life Support (CPR/BLS) certification
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing school
• At least 6 months of recent experience in a Nurse Practitioner role, ideally in home health
