Job Description

You'll be empowered to manage your own schedule and provide exceptional care to Medicare Advantage plan members residing throughout communities like Edison, Woodbridge, and Old Bridge.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Commitment can be set weekly or bi-weekly

Key Responsibilities:

  • Conduct thorough home health risk assessments for Medicare Advantage members
  • Evaluate medical histories, current conditions, and medication usage
  • Educate patients about their diagnoses, treatment regimens, and medication management
  • Identify gaps in care and provide actionable recommendations to improve health outcomes
  • Focus exclusively on assessmentsno treatment planning or prescribing required

Desired Qualifications:

  • Active Nurse Practitioner license within New Jersey
  • Current Basic Life Support (CPR/BLS) certification
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing school
  • At least 6 months of recent experience in a Nurse Practitioner role, ideally in home health
