Job Description

Deliver high-quality patient care as a Nurse Practitioner focusing on home health risk assessments for Medicare Advantage members across Shelby County, Ohio. This per diem opportunity offers a flexible, self-driven schedule that empowers you to choose shifts throughout the weekincluding weekendswhile making a real impact in the communities of Anna, Fort Loramie, Russia, Sidney, and the greater Shelby County area.

Schedule / Hours

  • Flexible shifts available Monday through Sunday
  • Self-scheduled shifts to accommodate your lifestyle
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities

  • Complete comprehensive annual health risk assessments in patients homes, each typically lasting about 45 minutes
  • Review and evaluate patients past and present medical history, chronic medical conditions, and medication use
  • Educate patients about their health status, chronic illnesses, and prescribed medications
  • Identify care gaps and offer clear, actionable recommendations to improve health outcomes
  • Conduct both routine annual assessments and post-discharge visits
  • Focus solely on assessmentno treatment planning or prescribing involved

Qualifications

  • Active Nurse Practitioner license in Ohio
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred
