Discover a dynamic opportunity to provide essential healthcare services to Medicare Advantage members through in-home risk assessments. As an advanced practice provider, you will play a pivotal role in supporting members overall health in multiple communities of Shelby County, OH, including Anna, Fort Loramie, Russia, and Sidney.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to suit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Review and document prior/current medical conditions, medication history, and overall health status
- Educate members on their medical conditions and medications during each visit
- Identify gaps in care and make recommendations for care plan improvements
- Focus solely on assessmentsthere is no requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Completed MSN from an accredited nursing school
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months NP experience preferred
Compensation for this role is $80-$90 per evaluation, with the addition of a mileage stipend and compensation for administrative time related to your assessments.
