Delivering clinical care in a dynamic home health setting, you will perform risk assessments for Medicare Advantage members throughout Miami County, Ohio. Your expertise will support patient wellness through comprehensive in-home evaluationsthese 45-minute visits could be annual health assessments or post-discharge reviews, at locations including Piqua, Tipp City, Union, and surrounding areas. With a flexible per diem schedule and self-scheduling options, youll have autonomy and variety in your work.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Minimal weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual risk assessments for Medicare Advantage members in the home setting
- Thoroughly review previous and current medical history, including medical conditions and medications
- Educate patients about their overall health, conditions, and prescribed medications
- Identify care gaps and recommend solutions for improved health outcomes
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current BLS CPR certification
- Board Certified by AANP or ANCC
- At least six months of recent Nurse Practitioner experience preferred
- MSN from an accredited nursing school
