Deliver meaningful care in the home setting by leveraging your expertise as a Nurse Practitioneroffering a flexible, per diem opportunity to perform home health risk assessments throughout Surry County, North Carolina. In this dynamic role, you will engage with Medicare Advantage plan members, providing critical healthcare evaluations that help improve patient outcomes and identify care needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule, allowing you to balance work and life
- Minimum weekly or bi-weekly hourly commitment required
Core Responsibilities:
- Conduct comprehensive annual risk assessments for Medicare Advantage members in various home settings
- Evaluate patients medical history, ongoing conditions, and medication usage
- Deliver patient education regarding medical concerns and medications
- Identify care gaps and offer evidence-based recommendations to optimize outcomes
- No responsibility for creating treatment plans or prescribing medicationsfocus solely on assessment and education
Preferred Qualifications:
- Valid and current Nurse Practitioner License in North Carolina
- Active Board Certification (AANP or ANCC)
- Up-to-date CPR certification (BLS)
- MSN degree from an accredited nursing school
- At least 6 months of recent Nurse Practitioner experience preferred
