Job Description
Deliver impactful healthcare services directly to patients in their homes by performing risk assessments for Medicare Advantage plan members throughout Bingham County. This per diem opportunity is tailored for a Nurse Practitioner with a passion for home health and interest in flexible, autonomous practice. You'll conduct focused health evaluations in areas including Blackfoot, Shelley, and Aberdeen.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Conduct annual home health risk assessments, generally lasting 45 minutes per visit
- Review and evaluate prior and current medical conditions and medication history
- Educate patients on their medical conditions and prescribed medications
- Identify care gaps and recommend appropriate follow-up actions
- No prescription-writing or treatment planning necessary
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board certification via AANP or ANCC
- Current BLS/CPR certification
- Six months or more of recent NP experience preferred
- MSN degree from an accredited nursing school
