Job Description

Embark on a rewarding per diem opportunity as a Nurse Practitioner, specializing in home health risk assessments. This position involves visiting Medicare Advantage plan members in their homes throughout Bannock County, including areas such as Pocatello, Chubbuck, and Lava Hot Springs. You will play a key role in supporting patient health and closing care gaps, all while enjoying a flexible schedule.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Self-scheduling to support work-life balance
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage members
  • Evaluate patients medical histories, current health conditions, and medication usage
  • Educate patients on health conditions and medications, ensuring understanding and compliance
  • Identify any care gaps and provide thoughtful recommendations to resolve them
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • At least 6 months of current Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing program

This opportunity provides competitive compensation, with $80$90 per evaluation, plus a mileage stipend and compensation for administrative duties.
