Job Description

Make a meaningful difference in the lives of Medicare Advantage members by performing comprehensive home health risk assessments across Shoshone County, including areas such as Kellogg, Pinehurst, and Osburn. As a Nurse Practitioner dedicated to home health services, you will enjoy adaptable per diem opportunities that allow you to determine your own flexible schedule.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Create your own work schedule
  • Weekly or bi-weekly minimum hourly commitment

Your Daily Responsibilities:

  • Conduct thorough home health risk assessments for Medicare Advantage members
  • Review and evaluate patients current and prior medical history, existing conditions, and medication profiles
  • Educate patients about their medical conditions, treatment regimens, and prescribed medications
  • Assess for gaps in care and deliver actionable recommendations to enhance overall health outcomes
  • No need to prescribe medication or develop treatment plans; the role focuses on assessment and education

Qualifications/Experience Needed:

  • Active and unrestricted Nurse Practitioner license in Idaho
  • Current Basic Life Support (CPR/BLS) certification
  • National Board Certification through AANP or ANCC
  • At least six months of recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
