Job Description
Make a meaningful difference in the lives of Medicare Advantage members by performing comprehensive home health risk assessments across Shoshone County, including areas such as Kellogg, Pinehurst, and Osburn. As a Nurse Practitioner dedicated to home health services, you will enjoy adaptable per diem opportunities that allow you to determine your own flexible schedule.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Weekly or bi-weekly minimum hourly commitment
Your Daily Responsibilities:
- Conduct thorough home health risk assessments for Medicare Advantage members
- Review and evaluate patients current and prior medical history, existing conditions, and medication profiles
- Educate patients about their medical conditions, treatment regimens, and prescribed medications
- Assess for gaps in care and deliver actionable recommendations to enhance overall health outcomes
- No need to prescribe medication or develop treatment plans; the role focuses on assessment and education
Qualifications/Experience Needed:
- Active and unrestricted Nurse Practitioner license in Idaho
- Current Basic Life Support (CPR/BLS) certification
- National Board Certification through AANP or ANCC
- At least six months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
