Job Description

A unique opportunity is available for a dedicated Nurse Practitioner to provide home health risk assessments to Medicare Advantage plan members within Shoshone County, including Kellogg, Pinehurst, and Osburn. This per diem role focuses on patient assessment and education with flexibility to create your own schedule.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Ability to self-schedule shifts
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive home health risk assessments, typically lasting 45 minutes each
  • Gather and review patients current and prior medical histories, ongoing conditions, and medication usage
  • Deliver thorough education to patients regarding their medical conditions and prescribed medications
  • Identify lapses or gaps in care, offering recommendations for follow-up and improved patient outcomes
  • Assessment-only focus: no responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner License in the State of Idaho
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least 6 months of recent Nurse Practitioner experience strongly preferred

Successful candidates will receive $80$90 for each evaluation, as well as a mileage stipend and compensation for administrative time.
