Job Description
Contribute to improving community health by performing home health risk assessments for Medicare Advantage members across Jefferson County, AR. As a Nurse Practitioner specializing in home health, youll play a vital role in supporting patient well-being through comprehensive annual health evaluations and post-discharge visits.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage health plan members in locations throughout Jefferson County, including Pine Bluff, White Hall, and Redfield
- Review and evaluate prior and current medical history, including medical conditions and medication histories
- Educate patients about their health conditions, prescribed medications, and preventive care
- Identify gaps in care and recommend steps to address them
- Complete documentation for each assessment (approximately 45 minutes per visit)
- Note: No prescribing or creation of treatment plans required
Qualifications:
- Valid Nurse Practitioner License in AR
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of current NP experience is preferred
