Make a meaningful impact by delivering home health risk assessments to Medicare Advantage plan members throughout Surry County. This per diem opportunity allows Nurse Practitioners with home health experience to provide clinical support directly in patients' homes, focusing on health assessments in areas such as Mount Airy, Elkin, Dobson, and more.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Self-scheduling: build a calendar that suits your lifestyle
• Choose a minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Perform thorough home health risk assessments, each lasting about 45 minutes
• Assess patients current and prior medical history, ongoing conditions, and medication usage
• Offer education and resources on medical conditions and medications
• Detect and document gaps in care, recommending next steps for the patient and care team
• No involvement in creating treatment plans or prescribing medications

Qualifications:

• Valid and unrestricted Nurse Practitioner license (NC)
• Board Certification through AANP or ANCC
• MSN degree from an accredited institution
• Current CPR certification (BLS)
• At least six months of nurse practitioner experience (preferred)

Compensation for this role ranges from $80$90 per completed evaluation, with mileage stipends and reimbursement for administrative time included.
