Engage with patients in a meaningful way by providing home health risk assessments to Medicare Advantage plan members throughout Wilson County. This per diem Nurse Practitioner opportunity allows you to make an impact within your community while enjoying flexibility in your schedule and autonomy in your role. Assessments are briefaveraging 45 minutesand may include annual health evaluations or post-discharge visits in settings such as Wilson, Elm City, or Lucama.

Schedule:

• Flexible shifts available any day of the week, Monday through Sunday

Key Responsibilities:

• Conduct comprehensive annual health risk assessments in patient homes
• Gather and review detailed medical and medication histories
• Educate patients on ongoing medical issues and medication management
• Identify any gaps in care and recommend appropriate next steps
• No prescriptive authority or independent treatment planning required

Qualifications:

• Valid Nurse Practitioner license in North Carolina
• Certification in Basic Life Support (BLS/CPR)
• Board Certified through AANP or ANCC
• Minimum of 6 months recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
