Experienced nurse practitioners are needed to perform home health risk assessments for Medicare Advantage plan members in Adams County, Ohio. This per diem opportunity allows you to set your own schedule and conduct visits in various communities, including West Union, Manchester, and Peebles.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
- Complete annual home health risk assessments, each lasting about 45 minutes
- Review and evaluate patient medical histories, including prior conditions and medications
- Educate patients regarding their health conditions and prescribed medications
- Identify gaps in medical care and recommend actionable next steps
- No requirement to develop treatment plans or prescribe medication
Desired Qualifications:
- Active Nurse Practitioner license (OH)
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum six months of current NP experience preferred
- MSN degree from an accredited nursing program
Compensation for this role includes $80$90 per completed evaluation, with an additional mileage stipend and administrative time considered.
