Work directly with Medicare Advantage plan members in various residential settings, supporting patients as they receive annual or post-discharge healthcare evaluations.
Schedule:
- Flexible shifts available any day, Monday through Sunday

Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate previous and current medical conditions and medication histories
- Deliver patient education related to medical conditions, medications, and healthy living
- Identify gaps in care and provide recommendations for appropriate follow-up
- Focus solely on assessmentsno development of treatment plans or prescribing required
Qualifications:
- Active Nurse Practitioner license in Kentucky (KY)
- Current CPR certification (BLS)
- Board Certification by AANP or ANCC
- A minimum of 6 months of current Nurse Practitioner experience is preferred
- Master of Science in Nursing (MSN) from an accredited institution
This role offers compensation of $80-$90 per completed evaluation, along with a mileage stipend to cover travel between patient homes and additional compensation for administrative time.
