Advance your nurse practitioner career by providing home health risk assessments within El Paso County, CO. In this rewarding per diem role, youll serve Medicare Advantage plan members, conducting in-person evaluations in residential settings. Locations include Colorado Springs, Fountain, Monument, and other communities in El Paso County, with each visit typically taking about 45 minutes. Assessments may focus on annual health reviews or post-discharge needs to ensure comprehensive wellness support beyond hospital care.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review medical history, active conditions, and medication regimens
- Educate patients about chronic and acute medical issues
- Identify any care gaps; recommend solutions to enhance patient outcomes
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of recent nurse practitioner experience preferred
