Job Description

Seeking a skilled Nurse Practitioner to perform home health risk assessments on a per diem basis in El Paso County, Colorado. This independent role supports Medicare Advantage plan members living in areas such as Colorado Springs, Fountain, Monument, and their surrounding communities. You will meet clients in their residences and use your clinical experience to complete assessments that help close gaps in care and enhance patient education.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule to fit your lifestyle
  • Commit to a minimum weekly or bi-weekly number of hours

Key Responsibilities:

  • Conduct in-home annual health risk assessments for Medicare Advantage plan members.
  • Evaluate patients prior and current medical history, conditions, and medications.
  • Deliver patient education concerning diagnoses and medication regimens.
  • Identify and report potential gaps or inconsistencies in patients care.
  • Provide recommendations for improving care coordination.
  • No requirement to create treatment plans or prescribe medications.

Qualifications:

  • Active Nurse Practitioner License in Colorado
  • Board Certification through AANP or ANCC
  • Current BLS/CPR certification
  • MSN from an accredited nursing program
  • At least 6 months of recent experience as a Nurse Practitioner preferred
  • Genuine commitment to patient care, communication, and education
