Elevate patient care as a Nurse Practitioner specializing in home health by performing comprehensive in-home risk assessments for Medicare Advantage members located throughout Henrico County, VA, including areas such as Tuckahoe, Short Pump, Glen Allen, and Highland Springs. As a valued contributor, you will make a direct impact by evaluating medical conditions and identifying opportunities for health improvement in a per diem capacity.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct in-depth home health risk assessments averaging 45 minutes per visit
- Review and evaluate patients prior and current medical history, existing conditions, and medication usage
- Offer education to patients regarding their medical conditions and prescribed medications
- Identify and document any gaps in care, delivering recommendations to improve patient outcomes
- Perform annual health evaluations and post-discharge visits without the need to generate treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Virginia
- Current CPR (BLS) certification
- Board Certification via AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of experience as a Nurse Practitioner preferred
