Experienced Nurse Practitioners are needed to complete in-home health risk assessments for Medicare Advantage members throughout Chesterfield County, with opportunities to serve residents in areas such as Midlothian, Chester, and Bon Air. This per diem role offers high flexibility with the ability to create your own weekly or bi-weekly schedule, and shifts are available any day between Monday and Sunday.

Key Responsibilities:

• Conduct comprehensive home health risk assessments, each typically lasting about 45 minutes.
• Review and evaluate patients current and past medical history, health conditions, and medication usage.
• Provide education to members about their medical conditions, disease management, and medications.
• Identify gaps in patient care and make recommendations to address them.
• Facilitate annual wellness visits or post-discharge evaluations as required.
• No treatment planning or prescribing required in this role.

Qualifications:

• Active and unrestricted Nurse Practitioner license in Virginia.
• Certification in Basic Life Support (BLS/CPR).
• Board Certification through AANP or ANCC.
• Minimum of 6 months of recent Nurse Practitioner experience preferred.
• Master of Science in Nursing (MSN) from an accredited institution.
