Job Description
Deliver high-impact care to Medicare Advantage plan members in Knox County, IN, by performing home health risk assessments as a Nurse Practitioner. This per diem role serves individuals in their homes across Vincennes, Bicknell, Monroe City, and surrounding areas, offering an opportunity to practice autonomously and directly influence health outcomes in your community.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose either a weekly or bi-weekly minimum hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for health plan members
- Review and document medical history, current health conditions, and medication use
- Educate patients about their medical conditions and medications, enhancing their understanding and compliance
- Detect gaps in care and offer actionable recommendations for improvement
- There is no requirement to formulate treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in Indiana
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- Minimum of 6 months NP experience preferred
- MSN degree from an accredited nursing program
Youll be compensated between $80 and $90 per completed evaluation, and receive mileage and administrative time stipends, ensuring your dedication is recognized and rewarded.
