Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members in Cass County
• Review and evaluate medical histories, current conditions, and medication usage
• Educate patients about their medical conditions and medications, ensuring understanding and compliance
• Identify gaps in care and make practical recommendations for improved health outcomes
• Focus on assessmentno requirement to create treatment plans or prescribe medications

Required Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Qualifications:

• Active Nurse Practitioner license in Indiana
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• Master of Science in Nursing from an accredited program
• At least 6 months of current Nurse Practitioner experience is preferred
