Job Description

Experienced Nurse Practitioners are needed to complete home health risk assessments for Medicare Advantage members throughout Cass County, Indianaincluding the communities of Logansport, Galveston, and Walton. This per diem opportunity offers highly flexible scheduling: choose your own shifts any day of the week and set your preferred hourly commitment, providing an excellent balance for your professional and personal life.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Build your own work schedule
  • Minimum weekly or bi-weekly hours

Key Responsibilities:

  • Perform in-home risk assessments for Medicare Advantage members (approximately 45 minutes per visit)
  • Complete annual health assessments and post-discharge evaluations as needed
  • Review and assess patients prior and current medical histories, medical conditions, and medication usage
  • Educate patients on their conditions, medication regimens, and general health management
  • Identify care gaps and provide recommendations for improvement
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Master of Science in Nursing (MSN) from an accredited program
  • Board Certification through AANP or ANCC
  • Current Basic Life Support (BLS/CPR) certification
  • Minimum of 6 months' recent experience as a Nurse Practitioner preferred
