Advance your career and make a meaningful impact on patient care by providing in-home health risk assessments for Medicare Advantage members in Monmouth County, NJ. This per diem opportunity allows Nurse Practitioners with a passion for patient-centered care to work within flexible scheduling while serving the communities of Middletown, Howell, Marlboro, and surrounding areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to suit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments, each typically lasting about 45 minutes
- Evaluate both prior and current medical history, review medical conditions, and assess medication regimens
- Educate patients on their health conditions and prescribed treatments
- Identify gaps in care, making recommendations to promote optimal health outcomes
- No requirements for creating treatment plans or prescribing medications
Preferred Qualifications:
- Valid Nurse Practitioner License in NJ
- Graduate of an accredited MSN program
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- At least 6 months of Nurse Practitioner experience (preferred)

Compensation for this position includes $80-$90 per evaluation, with a mileage stipend and additional payment for administrative time, ensuring your expertise and time are valued.
