In this per diem position, you will play a vital role in conducting home health risk assessments for Medicare Advantage plan members throughout Middlesex County, including areas such as Edison, Woodbridge, and Old Bridge.
Schedule:
- Flexible shifts available any day between Monday and Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual health risk assessments in patient homes (average assessment time: 45 minutes)
- Review and evaluate prior/current medical history, health conditions, and medication regimens
- Educate patients about their medical conditions and prescribed medications
- Identify gaps in care, and provide clear recommendations for follow-up or resolution
- No creation of treatment plans or medication prescribing required
Qualifications:
- Active Nurse Practitioner License in NJ
- Board Certification (AANP or ANCC)
- Current BLS (CPR) certification
- MSN degree from an accredited nursing school
- At least 6 months of recent NP experience preferred
