Deliver crucial healthcare assessments as a Nurse Practitioner specializing in home health for Medicare Advantage plan members in Perry County, MO. This per diem opportunity involves conducting comprehensive risk assessments in patients homes throughout Perryville, Altenburg, Frohna, and surrounding communities.
Schedule:
- Flexible shifts available seven days a week
- Set your own schedule with a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage health plan members
- Review and evaluate patients previous and current medical histories, ongoing conditions, and medication regimens
- Provide clear education and counseling regarding diagnoses and medications
- Detect care gaps and recommend follow-up or interventions as needed
- Focus solely on assessmenttheres no requirement to create treatment plans or prescribe medications
Qualifications:
- Must hold an active Nurse Practitioner license in Missouri
- Possess current CPR certification (BLS)
- Board certification through AANP or ANCC required
- Masters degree in Nursing (MSN) from an accredited institution
- At least six months of recent NP experience preferred
This position provides compensation of $80$90 per home health evaluation, along with a mileage stipend and pay for administrative time.
