Job Description

Deliver impactful care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage members across Peoria County. This per diem opportunity offers you complete flexibility in scheduling and the autonomy to manage your weekly or bi-weekly commitment. Shifts are available throughout the week, including weekends, and you can tailor your schedule based on your availability. During each approximately 45-minute visit, you'll support members at various locations in Peoria County such as Peoria, Chillicothe, and Bartonville.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Commitment based on weekly or bi-weekly hours

Key Responsibilities:

  • Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage plan members
  • Review and evaluate members medical histories, active health conditions, and medication regimens
  • Offer patient education on medical conditions and medications
  • Identify care gaps and provide actionable recommendationswithout prescribing or creating treatment plans

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN from an accredited nursing school
  • At least 6 months current experience as a Nurse Practitioner preferred
