Job Description
Embark on a rewarding role delivering comprehensive home health risk assessments for Medicare Advantage plan members throughout Escambia County, FL. This per diem opportunity offers significant autonomy, allowing Nurse Practitioners to create their own schedules, select shifts that fit their lifestyle, and care for patients in a home-based environment. The work primarily consists of brief, thorough visitseach assessment averaging 45 minuteswhich may include annual health evaluations or post-discharge check-ins.
Schedule:
- Flexible shifts available between Monday and Sunday
- Ability to create your own schedule
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform annual risk assessments for Medicare Advantage plan members in patient homes
- Review medical history, current conditions, and medication usage
- Educate patients on their medical conditions and medications
- Recognize care gaps and offer recommendations to close them
- No responsibilities regarding creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Current Basic Life Support (BLS/CPR) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months of recent Nurse Practitioner experience preferred
- Masters degree in Nursing (MSN) from an accredited institution
Additional Details: This position is compensated at $80 to $90 per completed evaluation. Mileage reimbursement and payment for administrative time are provided.
