Job Description

Deliver vital care to the community as a dedicated healthcare professional focused on home health in Fremont County. This per diem opportunity is ideal for licensed Nurse Practitioners looking for diverse clinical experiences and scheduling flexibility. By performing home health risk assessments for Medicare Advantage plan members, you'll play a key role in preventative healthcare for individuals residing in locations such as Caon City, Florence, and Brookside.

Schedule:

  • Flexible shifts offered between Monday and Sunday
  • Ability to create your own work schedule
  • Options for minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members
  • Assess prior and current medical histories, medical conditions, and medication usage
  • Educate patients on chronic and acute medical conditions, medications, and follow-up
  • Identify and address any care gaps, providing recommendations for improvements
  • Perform evaluations without the need to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in the state of Colorado
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of NP experience preferred
