Advance your clinical career by providing essential home health assessments as a Nurse Practitioner on a per diem basis. This role centers on completing risk assessments for Medicare Advantage plan members in various locations across Fremont County, CO, including Caon City, Florence, and Brookside. Each visit lasts approximately 45 minutes and covers either annual assessments or post-discharge visits. Schedule:
- Flexible shifts available Monday through Sunday

- Commitment to a minimum weekly or bi-weekly hourly requirement
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members.
- Thoroughly review and evaluate patients prior and current medical history, medical conditions, and medication regimens.
- Provide clear education regarding medical conditions and medications to patients in their homes.
- Identify gaps in care and offer recommendations to address these issues.
- No requirement to develop treatment plans or prescribe medications.
Qualifications:
- Active Nurse Practitioner license in Colorado
- Unexpired CPR certification (BLS required)
- Board certification through AANP or ANCC
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
