Exciting opportunities are available for Nurse Practitioners with home health experience to conduct risk assessments on a per diem basis. This role focuses on supporting Medicare Advantage plan members through comprehensive in-home health evaluations in Hancock County, including Greenfield, McCordsville, and Fortville. Each home health risk assessment typically averages 45 minutes and may include annual health reviews or post-discharge visits.
Schedule:
- Flexible shifts between Monday and Sunday
- Self-scheduled shifts for maximum flexibility
- Opportunity to meet a weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members.
- Assess medical history, current health conditions, and medication usage.
- Educate members on their medical conditions and medications to support better health outcomes.
- Identify and document care gaps, providing actionable recommendations.
- No requirement to create treatment plans or prescribe medications.
Qualifications and Experience:
- Current Nurse Practitioner license in Indiana.
- Valid CPR certification (BLS).
- Board Certification through AANP or ANCC.
- Minimum six months of recent NP experience preferred.
- MSN degree from an accredited nursing school.
