Job Description
Flexible per diem opportunity available for a Nurse Practitioner experienced in home health assessments. In this role, you will conduct comprehensive, in-home risk evaluations for Medicare Advantage plan members throughout Robeson County, with visits that may include annual health assessments or post-discharge evaluations. Each assessment takes approximately 45 minutes and allows you to make a positive impact on patient care within the local community.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments with Medicare Advantage members in their homes
- Review prior and current medical history, health conditions, and medication usage
- Educate patients on managing medical conditions and proper medication usage
- Identify gaps in patient care and recommend interventions for improvement
- No treatment plan creation or prescribing medications required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC required
- Current BLS (CPR) certification
- Master of Science in Nursing degree from an accredited program
- At least 6 months of current NP experience preferred
