Your role will focus on conducting comprehensive home health risk assessments for Medicare Advantage plan members located in communities such as Laurinburg, Wagram, and Gibson. Each home visit averages about 45 minutes and may include annual evaluations or post-discharge follow-ups. The emphasis is on holistic assessment and education, with no responsibility for direct treatment planning or prescribing medications.

Responsibilities include:

  • Completing thorough annual risk assessments in patients homes
  • Reviewing medical, surgical, and medication histories
  • Educating clients about their health conditions and medications
  • Identifying care gaps and recommending solutions to primary care teams
  • Maintaining accurate documentation without involvement in treatment or prescription

Desired Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certification through AANP or ANCC
  • Current BLS/CPR certification
  • MSN degree from an accredited school of nursing
  • At least 6 months of current NP experience preferred
