Job Description

Take on a meaningful per diem opportunity as a Nurse Practitioner specializing in home health, delivering risk assessments to Medicare Advantage members throughout Scotland County. In this role, youll visit patients in their homes for comprehensive assessments, dedicating about 45 minutes to each evaluation. With a flexible, self-directed schedule and locations that may vary across Laurinburg, Wagram, Gibson, and other parts of Scotland County, youll have the autonomy to manage your own hours while making a significant difference in your community.

Schedule:

  • Minimum hourly commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members.
  • Review and evaluate patients prior and current medical history, conditions, and medications.
  • Educate patients about their health conditions and medication regimens.
  • Identify potential care gaps and make actionable recommendations to resolve them.
  • Please note: This role does not require prescribing medications or developing treatment plans.

Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • Current BLS/CPR certification
  • Board Certification through AANP or ANCC
  • Minimum of 6 months current NP experience preferred
  • MSN degree from an accredited nursing program
