Schedule:

• Flexible shifts available Monday through Sunday
• Create your own customized schedule
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Perform comprehensive home health risk assessments tailored to Medicare Advantage plan members
• Assess medical histories, current conditions, and medication use to build a holistic health profile
• Educate patients on health management, medication adherence, and overall wellness strategies
• Recognize gaps in care, delivering thoughtful recommendations for improved outcomes
• Complete documentation efficiently; no requirement for treatment planning or prescription writing

Required Experience & Qualifications:

• Active Nurse Practitioner license in Illinois
• Current BLS (CPR) certification
• Board Certification via AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited program
• At least 6 months of recent nurse practitioner experience is preferred

You will be compensated with $80-$90 per evaluation, plus a mileage stipend and paid administrative time.
