The role involves traveling to visits throughout Peoria Countyincluding locations such as Peoria, Chillicothe, and Bartonvillewhere you will conduct comprehensive assessments for Medicare Advantage plan members in their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members.
- Assess prior and current medical history, review medical conditions, and evaluate medication history.
- Educate patients on their conditions and prescribed medications.
- Identify care gaps and recommend solutionsnote that no treatment plans or prescribing are required.
- Documentation and coordination of findings within each visit, typically 45 minutes per assessment.
Qualifications:
- Active Nurse Practitioner license in Illinois.
- Current CPR certification (BLS).
- Board Certification by AANP or ANCC.
- Master of Science in Nursing (MSN) from an accredited institution.
- At least six months of Nurse Practitioner experience (preferred).
