The role involves traveling to visits throughout Peoria Countyincluding locations such as Peoria, Chillicothe, and Bartonvillewhere you will conduct comprehensive assessments for Medicare Advantage plan members in their homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members.
  • Assess prior and current medical history, review medical conditions, and evaluate medication history.
  • Educate patients on their conditions and prescribed medications.
  • Identify care gaps and recommend solutionsnote that no treatment plans or prescribing are required.
  • Documentation and coordination of findings within each visit, typically 45 minutes per assessment.

Qualifications:

  • Active Nurse Practitioner license in Illinois.
  • Current CPR certification (BLS).
  • Board Certification by AANP or ANCC.
  • Master of Science in Nursing (MSN) from an accredited institution.
  • At least six months of Nurse Practitioner experience (preferred).
