Bring your nurse practitioner expertise to an impactful per diem role conducting home health risk assessments for Medicare Advantage members within Adams County, Illinois. This opportunity allows you to make a real difference, helping patients remain healthy and supported in the comfort of their own homes. Assessments typically take 45 minutes, and visits may be annual health checks or post-discharge evaluations. In this flexible position, your schedule can be tailored to your preferences, with minimum weekly or bi-weekly commitments. Candidates will be scheduled in various locations throughout Adams County, including Payson, Quincy, and Camp Point.
Schedule:
- Flexible shifts available Monday through Sunday

Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, current health status, and medication usage
- Educate members about their conditions and medications
- Identify care gaps and recommend follow-up steps as needed
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Illinois
- Current CPR (BLS) certification
- Board certification via AANP or ANCC
- Completion of MSN from an accredited school of nursing
- At least 6 months of recent NP experience preferred
This role provides competitive compensation$80-$90 per evaluation conducted, along with a stipend for mileage and administrative time.
