Bring your nurse practitioner expertise to an impactful per diem role conducting home health risk assessments for Medicare Advantage members within Adams County, Illinois. Assessments typically take 45 minutes, and visits may be annual health checks or post-discharge evaluations. In this flexible position, your schedule can be tailored to your preferences, with minimum weekly or bi-weekly commitments. Candidates will be scheduled in various locations throughout Adams County, including Payson, Quincy, and Camp Point.

Schedule:

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Review and evaluate medical history, current health status, and medication usage
  • Educate members about their conditions and medications
  • Identify care gaps and recommend follow-up steps as needed
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Current CPR (BLS) certification
  • Board certification via AANP or ANCC
  • Completion of MSN from an accredited school of nursing
  • At least 6 months of recent NP experience preferred

This role provides competitive compensation$80-$90 per evaluation conducted, along with a stipend for mileage and administrative time.
